Requirements:

• MD or DO degree from a fully accredited medical or osteopathic school; residency training in OB/GYN or relevant specialty
• Valid state medical license
• Board certification or board eligibility based on education and specialty
• Valid DEA license in applicable state
• BLS or ACLS certification upon hire (acute care) or within 90 days (ambulatory clinic)
• Facility-verified case logs demonstrating 30-40 deliveries within the past 36 months (personal logs not accepted)
