Notes: Provide comprehensive, continuous, and coordinated primary care services within a Patient Centered Health Home model, focusing on acute and chronic conditions for patients of all ages. The position requires working in a community setting with a primarily Spanish-speaking patient population, and the team includes support from Medical Assistants.

Required Exp:

• Minimum 2 years recent experience
• Comfortable with suturing; splinting preferred
